Community Connections

Research shows that long-term outcomes improve when individuals have a knowledgeable and experienced network to help them continue their recovery after discharge from inpatient or outpatient physical rehabilitation. Our goal is to help you achieve a successful recovery even after you leave our doors! We offer adaptive and medical fitness services and day recreation programming as an extension of our continuum of care with Sheltering Arms Institute. We are dedicated to connecting you and your caregivers with resources to support your needs throughout the recovery journey, whether now or in the future.

Community Connection Services

From diagnosis-specific support groups and connecting you with resources in the community to peer mentoring for you and your caregiver, community connections are focused on helping address factors that impact your health and providing needed resources to enhance your recovery journey to help you now that you have transitioned back into the community.

Support Groups

Support groups provide a combination of educational resources and a network of peer support. We provide support groups for caregivers in addition to several diagnoses, including stroke, limb loss, concussion, Parkinson’s disease, and aphasia. To view all upcoming support groups, please click here.

Peer Mentoring

Our Peer Mentor program provides members and clients the opportunity to connect with other individuals who have also experienced the physical rehabilitation journey first hand following injury or illness. Our mentors offer unique perspectives to help guide you as you continue your recovery journey at home an in the community.

Caregiver Ambassadors

Illness and injury are individual experiences that ultimately affect an entire family. Our ambassadors have lived experience as a caregiver and are here to support your well-being and success during your loved one’s journey by providing advice and compassion through one-on-one meetings.

Robust Referral Network

Through Unite Us, our community connections team is linked to an expansive network of valuable resources in your area that support identified social care needs. We efficiently connect you and your caregivers with verified organizations that support your continued integration into the community.

Continuing Education

Our EmpowerU series is a program of events designed to provide opportunities for you to grow, learn, and stay connected. As the program manager for the Brain Injury Program at SupportWorks Housing, I have recommended Sheltering Arms Community Programs for the past 15 years. Understanding the role of community connections

Community Connections is collection of programs and resources provided by Sheltering Arms Community Programs. We aim to identify, address, and improve your overall health by collaborating with partners who specialize in specific resources. The Community Connections team focuses on supporting your continued recovery journey by connecting you with the continued support services you may need.

Our key community partners are other non-profit organizations that provide crucial support to you and your loved ones. Representatives from our key community partners can visit you at one of our locations for in-person meetings to establish connections and relationships to support your return to community-based activities. Our team can also connect you with resources beyond our key partners as needed.

You or your loved ones are welcome to connect with our community partners at any time you identify a need for support or want to explore programs and services they offer.

Our Community Connections team can help recommend resources that best suit your needs. We can also meet with you or your loved ones to review resources and conduct screenings to find programs in your area.

You are welcome to contact our community partner agencies directly. Virginia Navigator and the Virginia Department of Health offer resource directories where you can search for support options. Alternatively, you can reach out to the community connections team and we will facilitate a referral to that organization on your behalf.

Our network extends far beyond our key community partners. Through a platform called Unite Us, we can search for and locate organizations and programs based on your unique needs and zip code to find verified resources that best suit you and your loved ones. Our goal is to help you and your loved ones learn about additional resources available to you so that you have the autonomy to advocate for your health and social needs.
