Discharge from the hospital is a significant milestone in your recovery! Once you leave the hospital, you may benefit from continued services in a less intensive setting. Prior to discharge, your rehab team will work with you to determine the appropriate next steps in your care and your case manager will help you with post-discharge planning.
Sheltering Arms offers a variety of post-acute programs, including outpatient therapy and home health, as well as recreation and fitness services, to provide a seamless transition for you and your family.
Outpatient physical, occupational and speech therapy are offered at Sheltering Arms centers conveniently located throughout Central Virginia. Most patients come several times per week for outpatient therapy appointments. Sheltering Arms physicians also see patients on an outpatient basis. Just like in the hospital, your outpatient care will consist of dedicated treatment focused on your specific needs and recovery goals.
Club Rec is a non-medically based program providing supervised recreational, social and wellness activities to help you stay actively engaged throughout the day. You may attend one to five days per week, depending upon what best suits your specific needs and schedule.
The Sheltering Arms fitness centers and therapeutic pool offer personalized attention in a comfortable setting. Equipment is wheelchair accessible and the staff has expertise working with individuals with limited mobility. This is an ideal environment for continuing the progress you have made in your rehabilitation programs.
