Community Health Needs Assessment

Posted on: July 8, 2019 by Jenny Lankford

As a non-profit hospital, every three years Sheltering Arms is required by the Internal Revenue Service to conduct a Community Health Needs Assessment (CHNA). By taking a minute or two to complete the survey using the link below, you will be helping us identify the needs of individuals with physical disabilities who live in our community. The results of these surveys will be used to help create an enhancement plan for future services and will be shared with the public.

Your feedback is very important to us and will go a long way in helping us continue to improve quality of life in the community we serve. The survey will close Friday, July 19. Thank you!
