For Healthcare Professionals
Rehabilitation occurs at a pivotal point in a patient's recovery process, and therefore communication between the referring health care team and the Sheltering Arms team is essential.
Referrals for inpatient services are accepted from physicians, hospitals, health care facilities and insurance companies. And, in an effort to make the referral and admission process as EASY as possible, we offer several ways to make a referral for inpatient rehabilitation. In addition to our local and toll-free telephone numbers, Sheltering Arms now uses both the ECIN and E-Discharge systems for patient admissions! Visit www.extendedcare.com/ or www.edischarge.com/ to utilize these services.
Our team of clinical professionals will work with you to ensure that your patient is appropriate for physical rehabilitation at Sheltering Arms before he/she gets to our facility. Prior to admission to one of our hospitals, a Sheltering Arms nurse liaison or physician will assess each patient to ensure that he/she can benefit from intensive inpatient physical rehabilitation services.
On an outpatient basis, you or a patient may schedule an appointment and simply bring a signed prescription on the first visit. Prescriptions are not needed for medical psychology services, or for fitness, pool, therapeutic recreation or the Club Rec program. For physician services, some insurers require a referral while others do not.
Please call us at (804) 764-1000 (inpatient), (804) 764-1001 (outpatient) or 1-877-56REHAB (toll-free) to make a referral, or for information about the referral process or services at Sheltering Arms. Our goal is to make the referral and recovery process seamless for you and your patients. We look forward to working with you to provide the best care possible!
